Contract for therapy between

Our usual therapy slot is

Therapy will usually be weekly for 50 minutes unless otherwise jointly agreed

Therapy cost available upon request, payable at session or via BACS

Our therapy agreement will depend on your goals. Your therapy goals will be reviewed after 6 sessions. This process will continue until we mutually agree your goals are met satisfactorily.

Please endeavour to maximise notice if you can’t make a session (at least 48 hours). Unless notified of cancellation (at least 48 hours’ notice) or cancellation due to an emergency you will be charged for full session.

Where two sessions are missed without explanation, regrettably you will be deemed to have disengaged.

 

Confidentiality

Your confidentiality is paramount. The only time this confidentiality may be breached is when you or someone else is at risk of harm. I will discuss this with you whenever possible.

Any information which identifies you will be stored separately from anonymised session notes and any other case information e.g. measurement tools.

Your case may be discussed anonymously with my clinical supervisor

Sessions may be recorded for discussion with my supervisor

Care is always taken by me and the centre to protect your identity and to comply with data protection regulations

If you have provided an emergency number at intake, then this will only be used in a genuine emergency e.g. if you are taken ill.

 

Behaviour

You agree to engage with the therapy process and not to attend under the influence of drink or drugs or to behave in a manner that could be considered aggressive.

You have been provided with my contact details; these are only to be used in relation to arrange our sessions. If you are having a mental health crisis help can be sought from the Samaritans (116 123) Breathing Space or contact NHS 24 or 999 for an ambulance

The client-therapist relationship is formal for the purposes of providing therapy. It is not permitted to become sociable and that includes social media

Feedback/disengagement/complaints

You will be asked for feedback at the end of each session. This is to ensure that the therapy is meeting your needs, and you understand what I am doing. If you don’t understand then this is your opportunity to highlight this.

If you wish to disengage from therapy then please let me know, giving me at least 24 hours’ notice
